if the quarterback throws a forward pass underhand and no one catches it is that considered a fumble or incomplete pass?

It is an incomplete pass whether or not it's thrown underhand as long as it travels forward toward an eligible receiver.

Incomplete pass. It doesn't matter how it's thrown, if it goes forward, it's a pass not a lateral.

If he throws it forward, no matter how he throws it, it's a pass. If he throws it behind him, it could be considered a lateral.
